This is another tragic example of how oversights or failure to follow safety processes in the workplace can have devastating consequences.  

A Lincoln based firm have been fined £20,000 plus legal costs for health and safety breaches leading to life threatening injuries suffered by a contractor who fell 3.5 metres through a hole in the floor.  The firm who specialise in fitting mezzanine floors had failed to guard off the hole or put a board up preventing anyone from failing through  - a basic safety measure.  The £20,000 fine relates to a criminal prosecution brought by the HSE and does not include any civil compensation award that the contractor may be entitled to which, given the nature of his injuries, could amount be sizeable.
